Booking form that works together with picture

Ok guys please read first before biding, i need a form where a client can select a certain product from a picture of a model, like a watch or bag or shorts e.t.c and on the side the form will be adding up the price like lets say i select a bag which costs $30 and also select a watch that cost $15 i will get a total of the price and products selected. Also in the same form there has to be an option of adding more details like lets say i selected a watch in the section there are two brands for the watch so the price will add-up depending on the brand selected. I just a need a booking form only not a website, i already have a website, so everything has to be happening in the the same page or if there is another suggestion i will listen. Also my budget is very small so stick between the proposed $10-$30 range
