Create simple checkout form on checkout page with thank you page (NO PAYMENT INTERGRATION JUST A FORM!)

I recently stopped using PayPal for my website payments. I would like to go manual and just enter credit numbers manually.
I need to add a form to the checkout page for credit card details (*Required). There is already an email/form system built into the website. To get a better understanding, visit the website and go the checkout page. The credit card details form will appear here.
Once the user submits the payment, the credit card details are email to both the user and us. The booking information is already sent. The new form details for the credit card info needs to be connected to the autoresponder.
This should only take 45 minutes or less to complete and is simple. I am a web developer. I just don't have the time to complete this using Ajax. The site is built using a script that i modified.
