Design in Flash

We are developing a product presentation in Flash. There is a section called Reporting. Where the person who will show the presentation to the clients will need to input certain data after the presentation is over. 1) Executives Name (*mandatory to enter this information by the executive) 2) Call Date * 3) Client's Name * 4) Designation 5) Hospital 6) Product Name * (Either we will make him enter or Will provide him with a dropdown) 7) Date & Time ( Should take default time of the TABLET on which he is showing it) 8) Location (IMPORTANT - Current location to stored in the database.. Maybe Google Map can be used to store the Current location in the database. When he submits it should be saved in a database which can be viewed later. Admin section in PHP : 1) Call list to be displayed with the same order as taken in the form 2) All the details taken from the form to stored and displayed in this list. 3) Should have the capability to Filter it with Alphabet, Date OR Product Name 4) Should be able to Email page or Call list Timeline - ONE WEEK Max
