Need custom proxy surfing wordpress plugin

I am looking for a plugin that will allow me to create a page and allow users to surf the web using a proxy.
They should be able to enter any URL, and while they are on the page they wanted to go to, there should be a top bar that has an ad banner of my choice.
Plus, there should be an option to change the IP address and keep surfing.
I have thousands of working proxies that we can use if need be.
This should be VERY user friendly and easy to use.
