Wordpress Plugin - A Slide In Pop Up Form - Needs to be Customizeable

Hi, I need someone who has developed a PLUGIN before for WORDPRESS. The PLUGIN must work for all new wordpress websites and be used on mac and PC computers. The animation is very simple. 1) A Full screen slide will appear from the left side and quickly slide into the screen. The background page that is being browsed will darken in opacity. 2) Then we can add forms into the next field which will appear as the next animation. The form can be linked by the programer to the auto responders such as: Get Response, Mailchimp, Constant Contact etc. The user can easily upload an image which must fit the dimensions specified by our plugin apx) 850 wide by 1080 tall for (1920 x 1080 monitors). There must be a way to showcase how the slider will appear on all 4 main devices: Desktop, Laptop, tablet, cell phone A similar option is available with the plugin 'Revolution Slider' I've attached the design below. Items that need to be customizable in the popup slider. 1) The user must be able to add a headline and change the color. 2) User must be able to add a large image. 3) User must be able to add fine text on the bottom of the image and place the text where needed. 4) User must be able to add field: Name, Last Name and Email and other fields if required.
