Build my form + Integrate to Mail chimp

When form is filled: 1-It will deliver data to multiple emails in PDF 2-It will save data to mailchimp list Procedure: First have to create custom form using HTML, CSS and JS. Will after that have to process data, data will be first sent to mailchimp, and popupulated data from same variation, will turn into PDF and send via PHP mail(); Procedure might be a little technical to understand but I had to share it with you because you talked about the progam I will be using.
