Need to build a membership form on website using "Licensed" forms. The form needs to be work with plugins which can allow for
1. Security validation from captcha
2. Age validation, standard number and email validation
3. Geography validation for Country, state and city (Auto updates)
Automated e-mails to be shared with subscribers who sign up. Email will contain a unique value generated through a formula and the e-mail template should be in a manner that can be changed (rich HTML)
Data Validation for subscribers: The application should validate in the database existing members based on parameters such as mail ID or/and mobile numbers to auto respond with a different email notification with previously entered data.
Import/Export to CSV/Excel functionality to the database to be built with control on the e-mail notifications. Form to build the same extract to Gsheets as a back up.
Form and all used plugins should be compatible with all prominent browsers and responsive to devices
Project needs to be built on a test bed for 2 weeks for through testing and sign off
