build a web app in php/mysql as per excel

hi i need someone to build a mobile responsive web app in php/mysql as per excel and deploy on my local, so all pc's on my office network can use that. it will be like a order form, from where staff will enter data. and another page for admin to view/edit/delete that. with some reports. that's it.
