Create a Popup to make a PDF download and insert in my database the email and more info about user

We need a small popup for our website
We want to show only one popup for our customers with the following aspects:
Show in many different languages (we have about 20 in our website)
1 popup for language
We have all the design ready
Client can click outside of popup or on X for the popup to disappear, and the popup shall not reapper for two days.
Popup need to be filled with name, email and a drop box
After filling all information client can click in “ obter guia” and start a pdf download
After download a new popup needs to be showed
Email validation
The data need to be stored in a new MySQL database with name, email, browser, Ip, size screen and other user relevant information
The data for drop box we can put direct in database table, but in some cases, we can disable one or another drop box option.
This script needs to be speedy; we are trying our website faster than 2 second load
