Create Qualtrics JavaScript multipart survey question

Need a simple proof-of-concept JavaScript item within a Qualtrics survey. We are evaluating how it handles multiple responses within an item. The user will select one of two displayed random numbers between 1-20. They will do this ten times. We need to collect the two displayed numbers and which one the user picked. Preferably we can submit each choice as a sub-response in a single survey item (as a separate column in the output report/spreadsheet/file). If not, we need to collect all responses into a single final response that is collapsed into a single column (not ideal). After the proof-of-concept is completed, we will move forward with multiple jobs that will be simple games that build on this work.
