I am trying to teach myself HTML5, CSS, PHP, SQL (using php MyAdmin) Using HTML/5, PHP, SQL, and phpMyAdmin I need help with: Creating a mock/draft database linked website to support the work of a theoretical GP practice; • The database entries such as name and address etc can be made up • The design should consider ERM, mapping and choice of attributes • Each entity (table) must have a primary key and at least two other attributes • Each GP will have many patients • Each patient could have several GP’s • For each consultation prescription details will be stored with the date they are written • Each patient will have a medical history with a list of conditions Basic requirements and functions: • Allow new patients to be registered • Allow removal of existing patients from the register • Display personal details of current registered patients • Display patients seen by any particular GP • Display details of any given patient via their NHS number or some other data • Display details of patients who have missed more than 2 appointments Essential A description of the code, how it works and notes within the code to enable me to understand it
