CODE: JAILU Experience: 5 modules on Addons Market, 4+ years of Prestashop experience. The website in the example doesn't seemt to be in Prestashop ( from what i saw the forms are on a website that is not in Presta, rather the commerce part of your bussiness is in Prestashop). #1 - if in Prestashop, develop 2 modules, one for managing proffesionals and clinics, with database fields for the forms, this way we get clear and manageable information that we can access both in their account and in search. The search module, 2nd module, would take information from the DB fields and allow a fully searchable page. Based on the forms complexion i would say there will be required 4-5 tables, at a glance. By making them modularized we get stability and the whole process can be managed and updated separateley from the other content. #2- if not in Prestashop then i ll use the same architecture in mind without modularizing the code and implement it in those specific pages. It also depends if the Account section of clinics and people is already made or that too needs to be built. There are a few questions of how things in the forms work with the Shop and what is the amount of inter-dependence. My bid is above your min-max limit because it is not an easy or fast thing to do and you will not get the quality and stability you need from a 100-150$ developer. This is a proffessional advice as i have many clients that need me to fix/clean their website from other people who did cheap work.
