I am looking for someone to build exactly 4 webforms that will take information from a user. The forms will have fields with textboxes, other with choice (should be configurable). Those forms should be protected only authenticated users should access them, once submitted a word/pdf form should be generated to be printed. A sample output is attached. I am looking for an extremly quick solutions and this can be done with with pure PHP/MySQl or using wordpress/plugins. Please note the user access is part of this deliverable. I am open for any other suggestion. I am looking for a 2 day delivery. Max budget for this is $100.
