WordPress plugin to select Auto Parts

I need a very simple WordPress plugin that embed a form using short code. This form must ask Make, Model, Part and Year of Auto Part. Selecting Make should dynamically load corresponding models, selecting model should dynamically load parts, selecting parts should dynamically load year. After submitting the form it should go to Get Quote page where a user is asked to enter his/her name, phone, email and zip and also a captcha. Then submitting the form should display a message that quote is received. Note that Make, Model, Part and Year are posts and when entered in URL they open corresponding pages for the post.
