You have to develop Html for File Named "Contact4".

When someone Click on Contact button , picture slides or Changes to Simple form as in File Name "Contact"

User will enter data of Name , Email & Offer & click Submit Button .

Once it is done , "Thank you" will appear there .

And data of form will be forwarded to xyz@gmail with the Subject: asd

Important Point:

I ll add link of Google link my self, just put a note in code where I should add

So put google left banner code go here Etc... Like that in code n style sheet
