Straight up before you read further: I've been through Google's version of this, not Meta's. I got an app published and brand verified through Google's OAuth review, which meant per scope justifications and the demonstration video they require. The two processes rhyme, and here's the part that carries over. The most common reason these come back is that the reviewer cannot reproduce the permission being used. They log in with the test credentials you supplied and try to reach the screen where that permission actually does something. If the video shows an admin view, or skips the login, or reaches a state their test account cannot, it gets rejected no matter how well the written justification reads. The second thing is how the justifications are worded. A technical reason fails review. A user facing one passes. "We request email so a teacher can have their roster export sent to them" works. "We need email to identify users" does not. Every permission wants its own sentence like that, tied to something the reviewer can see happening in the video. So I'd walk your test credentials myself first and confirm every requested permission is reachable from that account, before recording a frame.
