I need a website to conduct a survey that presents "fake" cookie banners before surveying

Hey everyone! I am currently researching the behavioral effect of different Dark Patterns in cookie banners on the acceptance rate of those banners. In order to do that, I wanna invite participants to an online survey, conducted on an unrelated topic. Before accessing the survey, participants are to be confronted with one out of four types of cookie banners. Roughly speaking, it's supposed to look like this: First page of the questionnaire is blocked by the cookie banner, which needs to be responded to in order to proceed. The banners should be designed as follows (I have a very specific but non-complex design in mind; more information provided upon acceptance): 1) baseline banner, basically a "best practice" design were all options are presented equally 2) aesthetical manipulation: one button looks signifantly more inviting (e.g. green instead of red) 3) hidden link: only an acceptance button is presented, the option to decline will be presented as a link in the text 4) sub-menu: declining cookies will only be possible via a "settings menu" (see sketch attached) Once the banner is resolved, the first page will ask for demographic information. A second and a third screen will be additional questions in the usual "agree on a scale from 1 to 5" manner. Then a fourth screen will povide a debriefing, presenting subjects with their initial choice of cooke selection and asking them about their "decision". I want to stress that I don't actually need to install any cookies in the subjects browsers (except for what is technically necessary to fulfill this project) - it's more about creating the apperance. Also, none of this needs to be fancy, but the cookie banners should definitly look authentic. As this is a non-commercial purpose, feel free to grab some code from existing consent management platforms, as long as the different kinds of banners are consistent in design. What is really important to me is the backend of this: I need to be able to access the data of the subjects which filled out the survey and clearly match which subjected selected which cookie and then gave which answers in the survey. Additionally, I will also need to be able to know about the time they took for responding to the cookie banner, as well as for the survey in general.
