Simple Website for a Summer School

I'm running a short summer school and I need a simple website that will:
- be visually interesting, based on images that I can supply. It will be similar in design & content to a leaflet that has already been produced
- display text describing the summer school (I can supply all the text)
- have an online application form (6 fields)
- take a payment, via paypal, when an application is made
I the website will have 6 simple pages, all of which can come from a single template. It can all be very simple content, and must look good on a wide range of computer/monitor specs.
This project needs to be completed in not more than 1 week from the end of the competition, and the winner will need to upload the website onto space provided by me.
I have uploaded a Word doc with all the relevant text & I will send all the necessary images to the winning bidder.
