Zoho Creator Terms and Conditions page

I have been using Zoho creator for several years and am good at most things but need some help with deluge.
I have created an app and need to add a Terms and Conditions / User License page. I need this app to initially display this page with a tick box at the bottom with and email submission field. Once the person adds their email address and accepts the conditions, then submits, the rest of the pages and reports become visible.
