I need a simple php form that collects one variable form the previous page.
i.e I have 3 different properties I am trying to rent short term, when the potential client decides to rent a property he/she clicks on a link to take them to the booking form. This value however has to be changeable and sometimes left blank as sometime the client will not come form viewing a property.
other features the form must have are name, address, phone, email ( I can give all fields upon start of project.
The form must also have date pickers that upon choosing a date in winter or summer will bring up different terms and conditions.
use of AJAX would also be good.
I am a web-designer that has an understanding of php. I am currently making a php, xhtml and css template that form must fit into, I will be wanting the form unstyled uploaded to a different server than the actually site and will style possibly transfer the form to the website myself. I have other PHP projects that need to be done, this could be seen as a simple first test project that will lead to other work.
