This project requires the development of an online form, based on 12 questions answered on a scale of 1 to 5 (Strongly Disagree to Strongly Agree).

The responses are scored based on a simple addition/subtraction formula and results presented online.

The formula separates responses to 12 questions into 4 areas:

- Communication

- Planning

- Roles

- Vision

And scores the respondent on each, and provides an overall score combining the 4 areas.

Form should be built if possible to embed into our existing Wordpress website; alternatively be branded/coloured to match screenshots to be provided.

Must be designed to be an attractive UI and must require a name and valid email address to retrieve results online. Results and respondent details must also be emailed to our site admin
