Recode Form in bootstrap (html5 & js)

I have a form that needs to be recoded in bootstrap. we have a wireframe as seen here

we also need to copy some js functions from another form we had running. you should be able to copy the js functions without a lot of work. here is a video of the other forms js functions

Should take just a couple hours to get this knocked out. The bootstrap form will initially be unstyled, we want to check raw html and functions before we start themeing it.
