Convert PDF to a formatted MS Word document & extract images

I am in the process of converting a paper-based psych test into an online test. For this project to proceed, I will require text and images extracted from the provided PDF document. The PDF document has approximately 400 pages. The PDF document needs to be converted to a properly formatted MS Word Document. The PDF document contains questions that may have images as answers. These images are to be extracted to files in a hierarchical folder structure that can be easily referenced from the Word Document. Each image is to cropped of bordering white-space. For example: Question 7. Which of these shapes best completes the pattern <image: test_1/question_7/[login to view URL]> a) <image: test_1/question_7/[login to view URL]> b) <image: test_1/question_7/[login to view URL]> c) <image: test_1/question_7/[login to view URL]> d) <image: test_1/question_7/[login to view URL]> The resulting Word Document must be almost exactly the same as the original PDF; change in content or poor layout will not be accepted as completed work. A detailed video tutorial will be provided to assist with any questions. Once phase 1 is complete, there may be more opportunity for work. We are using LimeSurvey as the engine.
