Brochure Cover Page and Page Template

I need to create an A4 half-fold brochure. It is for use in the medical and pharmaceutical industries - in particular the cover needs to consist of an idea to show the change in quality of life after surgery and the loss of diabetes. (so a before/after concept is required on the cover). I'd like the cover to portray this and then to develop a generic look for inside pages of the brochure. I am open to the purchase of stock photos if required. I've included a past example of a brochure so you can see colors, logo, etc. (the design of this example brochure is too simple though - it's just referenced for colors). I want to receive the file in InDesign format so I can add my own text content. So this is just requesting help with developing the layout. Thanks,
