Create a 5-10 Second Video Intro

Hello, I'm looking to have a killer 5 to 10 introduction clip created using the text "Commission Formula" with a sub-title "Next Level Affiliate Marketing" in the intro of the video. That'll give you an idea of the kind of effects I like - this shouldn't take most video guys very long to do. Video must be compiled into .mov format @ 640x360 and also in various other sizes so that I may re-use it in different instances. Person who can create this in shortest time & lowest bid wins.
