Build a website that stores survey data and displays them on a dashboard

Requirements: A dashboard that is accessible via web and can be displayed on Mobile and Computer. A survey form must be created with inputs (inputs should be customisable by admin and can add as many fields as possible). The data collected from this survey must be sent to a database. This data will then be collected and displayed on a dashboard in the admin panel. Back End: 1. Admin panel that displays tables and charts based on surveyed data (more details in Front End) 2. Admin panel must be in the form of a dashboard that has a responsive, fluid and minimalistic design. This dashboard must display custom tables and charts based on the data that has been collected via survey. 3. Admin MUST have the capability of creating as many categories as possible. Each category must have its own unique survey (that is customisable) and sends the data to the database for display in the dashboard. 4. Admin MUST have the capability of creating as many surveys as he likes and assigning survey answers to specific category. 5. All of the above must be as fluid as possible. No static websites, it should be dynamic. 6. Admin must have capability of adding custom image logo in top left and top right of the dashboard admin panel. As mentioned above, the admin must be able to create as many surveys as he likes and assign the survey data to a certain category. 2. Each survey created must generate a link in order to share with the public. 3. Survey must be fully customisable and admin should be able to add as many fields as he wants (text entry, dropdown, tick box, etc.) 4. Design must be fluid, minimalistic and capable of being displayed on all tables, pcs, mobiles, etc. 5. Admin should have capability of adding custom image logo in the top left of the survey and in the bottom centre (at the end of the page). Details can be discussed further once bidding process begins.
