Adding Javascript checking to an existing form.

I have an existing form. I just need javascript to be added to the form. The form has 10 questions with yes, no, not sure options as radio forms. I just need a javascript check to be written in the form to do the following when the user clicks on submit. If all the answers = yes the user will be sent to one page when they click on submit. If any question is answered with no or not sure the user will be sent to another page when they click submit. The answers in the form will not be stored, collected, or passed on. If you need the html form to add your javascript I can provide that once I award you the project.
