Android App - Forwarding

I need an application written that will allow the host device to forward phone calls and SMS messages to a designated recipient mobile. It will be a pretty basic app that just has a couple of settings to enable / disable forwarding for both calls and SMS, and accept phone numbers for the recipient either via a text input or contact selection. - The host device should show an icon in the top toolbar when any forwarding is enabled - The project will be provided and the code should be neat and expandable
