Create the Functionality Layout for an App

I would like someone to create the functionality layout for an APP I eventually want to have created for iphone, android, ipad, blackberry, etc. The app would allow people to input their availability and skillsets for work. There are 7 fields that the person needs to input data in and one optional comments field. 1. Industry (can check all that apply) - Medical Device - Pharmaceutical - Biotech 2. Skillset - I Will provide the list of skillsets that a person can check if they have expertise in it. 4. Dates Available - Select the start date and end date that they can work. 5. Hourly Rate 6. Name 7. Contact information (Email and phone number) 8. Comment section. All this data needs to be housed somewhere for me to access. For example: When I have a project and I need a quality engineer in the medical device industry that needs to start next week for a 3 month long project I want to be able to go to a database that has all candidates data that was submitted via the app. I want the candidate to also be able to change dates if something comes up and their availability changes. I want the database to automatically be updated. I would like to be able to sort the database by skillset, date available, etc. An Excel-type spreadsheet format will be sufficient.
