Create a PDF form and online data entry form

We need a form to be created which can be completed online or by pdf. The form needs to be submitted so it comes in pdf format with all the data aligned and organized like the original form. Some data is mandatory. The online form must be in HTML style for data entry submitted so it comes by email in pdf format. User access and profile • The user will have a login page where they will have access to a profile area • Profile area must include typical account adjustments to email, personal data including first and last name, company data including name, address and contact information (email & phone) • The profile information will auto populate when the user completes the form online but shall be editable. • Password reset option is necessary also Admin access and features • Administrator will setup username and password for each user • Admin will have access to all account information and completed forms • All data should be exportable by user while keeping separation between general contact data and forms (all data must be exportable in csv) • All forms must be available to print in pdf format from admin panel * Online portion of form needs to be in HTML or PHP format online so we can easily adjust field labels in the future if needed
