Yes, I think Spiceworks should be able to work for you. There are really two ways you can do this. As you described you can have a single installation of Spiceworks setup, and have users remotely connect to the installation via the User Portal web service. I would, however, recommend a slightly different setup: Install a "remote collector" installation of Spiceworks on each network where users need to enter support calls. Each of these remote collector installations can have a unique mailbox and User Portal layout. As users submit tickets (either via email to their site's mailbox, or via their site's User Portal), the tickets are automatically synced to your central installation of Spiceworks. You work the tickets from your central installation of Spiceworks, and the updates and sent back to the remote collector installation where the appropriate mailbox sends a notification of your reply. This allows you to use Active Directory authentication at each site, targeting a specific AD server, and even targeting a specific Active Directory OU.
