Customize BreezingForms PDF Output

We need to customize the PDF attachment AND export in BreezingForms component on a joomla site. The form's field sequences in the pdf output must be kept. Attached is the basic form we have prospective employees fill out so this is what we would like to keep it looking like. there are 47 fields total on this application, some are not required but we would still like to keep the fields on the export even if the prospective employee did not fill them out.
