Control Panel (Small Job to start with)

Hi I need a control panel for a site, where users can register, login etc. I need the form to gather data as follows: Name, Address, Postcode, Email Address, Password (for login), Phone Number, Payment Option (Cash, PayPal, Cheque, Standing Order) I also want the admin to be able to mark off when payment is received via the admin panel. It will then show in the users login area, that its been marked down as paid. I will be adding to this, making it a bigger job once this is done. I have alot of projects to be completed, my max for this is $50. It'll take 1-3 hours max to do, i could do it myself. But im tied up! Would rather build a relationship with another programmer, designer... Without being messed around, like i was before. Thanks.
