Adding Image Upload to Customer Order Form

This is a very simple shopping cart, the cart I need the feature on is at: [login to view URL] There are products that allow for customization, but we need there to be attributes that allow image upload. The images can either FTP to the server, or be sent via email. This is an open source cart, it does require a license to function but I can provide the files & localhost license if you want to run on your local machine. Standard PHP & MySQL database. I need this to be setup as an add-on that I can install myself.
