Create a Functioning Form

Create a Web application that allows participants to sign-up and administrators to view/analyze the results. This project has 3 parts as follows: 1. Create Survey Include the questions (What is a Clinical Study?, etc.) from the top of the current form on a right sidebar. The following fields are required: • Last Name • First Name • Zip Code • Email Address • Phone Number • Gender • Date of Birth All other fields are optional. All fields must have proper validation checking using Javascript. (pop up javascript validation) Note - if date of birth indicates that the user is less than 18 years old provide an error message stating that the parent must register. 2. Setup database and store the form results Setup a MySQL to store the form results. Use PHP to process the form and submit the results. 3. Administration Area Create a password protected administration area (set the default login ID and password to admin and admin so I can access it easily). This area must contain the following functionality: • Allow administrator to create/update/delete other administrators • Allow administrator to create/update/delete trial participants • List all trial participants in a table format (use zebra stripes) • Ability to sort trial participants by zip code, date of birth, gender, and/or condition. 4. JQuery feature Include at least one other JQuery feature on the site (can be on the form site or administration side).
