Custom plugin form for Wordpress

Hello I need a custom form for a Wordpress site + control panel (plugin). Form is used when customers creates a quote on some work (graphical). I have found a form i really like. I have attached screenshot from 99designs.com. If you want to see that form live you can go to [login to view URL] and go to step 2 in "Logo Design Contest". (I recommend you do that to see requirements.) Form needs the following: - Normal text input - Dropdown selection - Checkmark selection - Select samle function - Upload file function - Scroller function (this should return an integer between 0 and 100. 0 if scrolled to far left, 50 if in middle and 100 if scrolled to far right) When a form is submitted an email should be sent to a specified email and order/submission should be stored in Wordpress admin. I would then like to be able to see all submissions from admin panel / control panel. Only one function is needed in admin panel, that is to mark a submission as pending or completed. This is only for my own needs. No further functions will be needed in admin panel. What i would like for you is to create a similar form as seen on [login to view URL] with a working backend. I will be able to edit code myself to suit it for my own needs. Payment: No milestones will be released before i see a working form/control panel. I imagine you develop on own hosting. When are done you will give me URL to form and access to admin panel. Then i can test. Milestones: 1st milestone created when you start the project. 2nd milestone created when i am able to test on your server. I will release 1st milestone when i have checked form + admin panel to fill my needs. 2nd milestone will be released when transferred and tested ok on my server.
