Address verification for my website

A get paid to surf site script editing I need the script to be edited. when you signup my script does not allow you to enter a home address and i started to edit my script and ran in to a rock i am not very fimilar with php to edit any further What i edited is .. It now has a spot to add the address and city state, etc on the signup page, edit page (in member section), and admin section. ok so it has the space BUT it will not store the info on my data base i did however add the table names in the mysql database so it has space to be saved and as far as i thought the script pointed to the correct tables, but it will not store the new info. it is most like a simple editing mistake. I need the script to point to correct tables to safe the address info of my members. i also need it to be able to edit it on both the admin side and the members side.
