Hello I am Adobe Livecycle developer working for HSBC. I create interactive forms for bank and have more than 3 years of experience in this field. I have very clearly understood your requirements and I can handle this job. Inorder to handle your requirement you need an XDP form a normal pdf designed won't be able to save the data, the xdp forms have xml embedded with the pdf form and the data entered can be submitted via email. I can offer you one more option here: Save functionality - User of the form will be able to save the form WITH DATA ENTERED on their desktop and they can conveniently submit the form via email once they fill out all the information. Also I would like to understand where you would like this form to be available?? If develop the form, email scripting and save functionality and provide you the reader extended form will you be able to place the form on your server for the users to access via the url?
