ASAP Convert Word 2016 Fillable form to fillable PDF OR Make the current Word version work!

I created a "fillable" form in Word2016, but I can't get all of my content controls to work. It is only one page and no design work is needed. The drop downs and date selection areas work. But, I can't get the checkboxes and enter text boxes to work at the same time. I want a form that can be filled out by a remote employee and then emailed to the main office to print and use in house.
