i'm looking for a custom psychological assessment report template that allows me to streamline my reports. Specifically, i'm after a word template that enables me to enter client information and assessment results, and for particular content from tables (identifying information and assessment results) for example from identifying info - client's name, age and from results, scores, percentile ranks, range, and descripive category (average, high average etc) automatically enter the body of the report... i will upload both the assessment report example with the tests and score descriptions as well as a template with just the headings so it is clear what i'm asking! The tool im using is Wechsler Adult Intelligence Scale IV
