Build a javascript program for Qualtrics

I'm looking for someone to design a javascript or HTML program that can be embedded into a Qualtrics online survey. I want to just copy+paste the code into the javascript or html box. The program will be a 2-4-6 task, also known as the "Rule discovery task." Here's an example, and I'd be perfectly happy if it worked exactly like this: [login to view URL] The program should save the text responses and allow the user to continue on to the next page where I will have survey questions.
