Android simple app

I need an Android App with this features 1. Secure login [login to view URL] (about 200) 2.1 Questions with text answer 2.2 Questions with yes and no answer 2.3 Questions with yes and no answer + Why answer 2.4 Questions with text answer + picture atached (take a picture or select from album) 3. All questions have to be saved in a csv file locally to be uploaded later to the server 4. All the series of questions have to be saved as a project web base 1. I need a web admin panel to download each project with pictures in a zip file We Provide 1. Server 2. Graphic design 3. Questions The app have to be private, no one else can install it
