Website Flash with PHP work

We need a flash website made identical to :[login to view URL] ( clone it and update it with attached .doc updates )with customizations as per the uploaded doc. Also we need the application forms to save the data in a mysql db , and that someone can login and view the applications , by date . Also all the links on calcualtor page need to be made into direct links to certain pages we will specify. Also the email contact forms , and other forms must auto email a specified address.
