I need to get a contact form that is almost the same as this one: [login to view URL] (the planner). I need the form to be able to save to a PHP database on my servers and to send me an email. As long as the FUNCTIONALITY is there and the connection between the database and the form is done i can take care of the CSS. The winner will have less than 24H to finish the project
