Need some one to create two google forms documents

I need some one to transfer a word document and a publisher document into an on line form. Once completed; We musty be able to create and fill the these forms in on line and have them saved to an on line workspace like a database. Must have simple functionality The forms are not very complicated or difficult to understand., Must have attachment facility. the form must also have a customer approval sheet with a digital or on-line signature. storage in a PDF format is preffered
