URGENT: Joomla forms layout (Joomla forms + CSS)

Hello I already have a joomla based website and i want to integrate few forms into it. I've purchased a module from [login to view URL] . The module works perfect. But the CSS of the form is ugly. I want somebody to work on the CSS part alone. My form should look like this [login to view URL] . Since i purchased everything which i need to make the forms, iam offering just $30 for this job. If you're work is good i'll award more projects to you. The work will not take more than a hour for joomla / css professionals. Please do not bid if you have no idea about Joomla + forms.
