Auto generate Documents

I'm looking for someone that can take a form and generate / populate numerous different company documents with the information inserted into the form online (via outside sales person) for a new customer. We have several different forms that need to be individually filled out and signed by the customer in the field at this time. It doesn't seem to me that it would be that difficult to do, just not something I have time to research how to do myself as my background is in mid-size mainframe programming. I would like this program to: a) Store customer information b) Populate all of the different documents required based on the info inputted by the sales individual on the initial screen (Name, Address, Phone, etc) c) Email completed and signed contracts and forms directly to company email to be processed at the office.
