Book Now Page Modification

I need some changes to my existing website book now page ([login to view URL]). The changes are described below. Its a very simple change for any one with PHP experience. Please provide a proposal for your work and time frame and ask any questions you might have. Please go through brain tree docs to see how it can be integrated to the website. Changes required: Confirmation Display on Booking Payment Integration with Braintree Customer Profile with user name and password login with last purchase and option to add /remove extras Date and Time Field validation How often (one time, monthly, bi-weeky & weekly) option the %off should be calculated in background and not mentioned in the display with 5% off for monthly, 10% off for bi-weekly and 15% off for weekly. Pricing (hourly maid option should also be provided- with number of hours and number of maid as selectable and 2hrs minimum $28/hr rate) More Professional display (with Icons) Payment options ( Cash, or credit card)
