Build a very simple exam answer sheet webpage with login function

The website is required for submitting the test answers by a student. I need a very very simple webpage and nothing complex with admin control to change few parameters. This website is for personal use only so no need of security and encryption, just simple coding with the help of SQL database. A very basic layout is sufficient. Only the answer sheet has to be designed properly so that there is no misalignment. I want this to be completed as simple as possible as I need it little urgent. If the work is good, i will use the same person for further developing it in future. Login Page and Answer Page: The index or home page of the website consists of a login page. After login there must be only one page which must contain the answer sheet similar to the one in the image with questions equally divided in two or three columns depending upon the number of questions (Usually the questions varies from 60 to 120. The page consists of question numbers and 4 or 5 radio buttons for answers. For each question there must be one 'X' button to clear the response. On top left and right a remaining mins timer must be running which is set for a predefined value in the admin page. At the bottom there is one button named 'UPDATE ANSWERS'. Functioning: The user must be able to select either of the 5 options and also must be able to clear the response with the X button. When the user clicks the UPDATE ANSWERS Button a dialog box must ask for confirmation with OK or Cancel and the answers they have filled must be saved as a text file in the following format. Question Number, 65, Answer, A (America) Question Number, 5, Answer, B (Brazil) Question Number, 58, Answer, E (England) Each time they press the update button the latest updated entries must come on top of the text file and those answers which are filled must be locked in the answer sheet so the user cant modify or clear it anymore.. At the end of the timer a dialogue box must appear stating that the time is over, but the user must be able to still work. IMPORTANT NOTE: [login to view URL] progress of the user must be saved permanently until the responses are cleared by the admin. The user must be able to login again and continue from the point where he left. So every time he answers a question it must be saved in the table irrespective pressing the update answers button. [login to view URL] is another important thing, sometimes two or more persons will work on the same login in the same answer sheet, so the answers must reflect automatically in both the screens (This is necessary condition). ADMIN PAGE: In the administrative page (separate login page) I must be able to change the following settings. 1. Option to select number of questions (Number field to enter the value) and number of answers for the test (Radio buttons to select option either 4 or 5) (Note: If this kind of option is more time consuming for you, then leave it, just keep the default as 5, we will see it in future) 2. Input Starting Question Number (Number field to enter the starting number) - Because sometimes the [login to view URL] doesn't start from 1. 2. Editing Student login details, only two fields Username and Password. (There is only one student, so no need of multiple logins) 3. Set the timer in HH:MM format or whichever is simple to do. 4. Two buttons to Clear the responses in answer sheet and clear the text file. You can use either windows or linux hosting, you have to host the website in a best free hosting service with a temporary name of your choice. After the completion of the project, all the files has to be given without any restrictions to editing. I have attached the images for explaining the layout of the website.
