I recently worked with a Fire Station where I added a job application form to their website. They were using WordPress and reached a point where they were limited by the plugins they were using. I set up access to a database so new applicants information would be saved there. I also set up verification for email addresses, names and phone numbers to avoid sql injection as well as to keep the formatting the same. For example with the phone number. Lets say they type in 1(555)-345/9090 The first thing it does it gets rid of all non alpha-numeric characters. Then, if there are 11 digits it checks for a leading one and gets rid of that. Then it is formatted to add '-' after the first two sets of 3 characters. So the result that is saved in our database looks like this 555-345-9090 Which can be done any way you like. The names were checked for symbols and length. The email address was verified to actually be an email address. These verification are completely customizable and can pertain to anything. I'm not familiar with the plugin you are talking about but if nothing else we could add our own fields with it (or replace the plugin all together) using PHP. I will need your username/password for the FTP server that your domain provides to you. I'll also need the username/password for your WordPress admin page which will be a different set of credentials. Feel free to contact me if you have any questions. Best of luck to you.
